COVID-19: School and Child Care Guidance

If your child has a possible COVID-19 symptom, please complete the Ontario Ministry of Health COVID-19 Screening Tool for Children in School and Childcare before seeking an appointment at our office. The Ministry of Health guidelines were updated on February 27, 2021.

If after completing the screening questions you are not clear about your options, or if you are concerned about your child's health, please call our office to book an appointment and have the screening tool handy for your appointment. Physicians and nurse practitioners cannot provide notes for return to school when a COVID test is indicated based on the screening tool. School notes are only provided in the rare circumstance that a chronic medical condition is causing the symptom.

To book a COVID-19 test, visit the Brantford County Health website at or call 519-751-5818.

Doctor's notes:

As per the Ontario Medical Association: "doctor's notes for clearance to return to work, school, daycare or recreational activities are not an appropriate use of primary care resources, especially while COVID-19 causes unprecedented strain on the health-care system..." The Ontario Ministry of Health also recommends against requiring doctor's notes to clear students and staff to return to school after suspected/confirmed cases of COVID-19. Instead, we recommend that individuals and parents complete this COVID-19 Self-Attestation Form to assess whether it is safe and/or prudent to return to work, school, daycare or recreational activities according to COVID-19 clearance guidance from the Ontario Ministry of Health.
